Student Forms
The majority of student forms are available on the Office of the Registrar website, including forms for: FERPA, graduation, adding/dropping majors and minors, name changes, registration-related (add/drop, ISY, internships, overloads, pass/fail, RA/TA, etc), and withdrawing from the university, among others.

Curricular Matters
Chairs and Program Directors will conduct a syllabus review of the syllabi for the courses offered for their program at least once a year. The syllabus review worksheet has been provided for tracking this task. Once completed, the worksheet can be submitted using the Syllabi Review Form.

Staff Recruitment, Hiring, & Evaluation Information
The Department Chair will have the responsibility to conduct an annual evaluation of the administrative staff person(s) in their department. When the administrative staff position is shared across departments, the responsibility to evaluate the staff members is shared among the chairs of the departments involved.

Finance Policies and Purchasing Policies can be found in PolicyTech by navigating to MyStetson and selecting the “University Policies & Procedures” tile (or clicking this link). Then, expand “Finance” from the left-side menu by clicking on the +. You will then see the option to click on either ‘Finance’ or ‘Purchasing’ and the associated policies.
