The following form is used to notify the Grants Office of the intent to submit a grant proposal on behalf of Stetson University. Please refer to the First Alert Policy prior to submitting the form.

The following policy, forms, and certifications are to be submitted to the Grants Office during the grant application process and annually:
- Policy on Financial Conflict of Interest in Externally-Funded Research
- Disclosure of Financial Conflict of Interest Form
(for all Principal Investigators and Senior Personnel of external grants) - U.S. Public Health Service (PHS) Disclosure of Financial Conflict of Interest Form
(for grantees of PHS agencies such as NIH, HRSA, SAMHSA) - NIH specific training and certification for Financial Conflict of Interest
All Principal Investigators and/or Project Directors of grant proposals that involve research on human or animal (vertebrate) subjects, must contact and seek approval from the appropriate campus review boards:
The following procedure is used to collect signature approvals for submission of a grant application on behalf of Stetson University. The Grants Office staff will complete this form with information provided by the Principal Investigator or Project Director of the grant.
