Insurance Information for Students

As with most colleges, all students enrolled at Stetson University are expected to have adequate health insurance to cover them while attending the university. In an effort to make affordable coverage available to those who may not have any insurance, as well as to those who have limited insurance, a campus student accident and sickness health plan has been developed to provide quality health care to Stetson students at a reasonable cost.

Stetson University will not automatically enroll students in the student insurance plan. Students must submit an insurance enrollment form and payment to the insurance agent in order to be enrolled in the plan.

Enrolling in the Student Plan

Please go to http://www.CHPstudent.com and choose "Stetson University - DeLand/Celebration Campus" from the drop-down menu at the top of the page to enroll in the student insurance plan, review the flyer, look up in-network medical providers and obtain other important information.

The enrollment grace period for the 2015-2016 Annual and Fall-Only Plan ends at midnight on September 12, 2015. Students will not be able to enroll in this plan after that date and time unless they have a qualifying event.

Policy Dates and Costs

Annual Plan

  • Coverage Dates: August 12, 2015 to August 11, 2016
  • Cost: $2,590 for annual plan providing coverage dates above
  • The enrollment grace period ends at midnight on Sept. 12, 2015.

Fall Semester-only Plan

  • Coverage Dates: August 12, 2015 thru January 6, 2016
  • Cost: $1047.00
  • The enrollment grace period ends at midnight on Sept. 12, 2015.

NOTE: If you choose the fall-only option and will continue as a student at Stetson University in the spring semester, you will need to enroll in the spring/summer plan in January 2016 in order to continue your student plan coverage throughout the remainder of the academic year.

Spring/Summer-only Plan (Available 1st Week of January 2016) 

  • Coverage Dates: January 7, 2016 to August 11, 2016
  • Cost: $1,543.00
  • The enrollment grace period ends at midnight on Feb. 7, 2016.

Summary of Benefits

  • PPO Accident and Sickness Health Plan Coverage Limit: No overall max limit
  • Annual Deductible: $150 In-Network/$400 Out-of-Network
  • Coinsurance Coverage: 80 percent In-Network/60 percent Out-of-Network
  • No exclusions on pre-existing health issues
  • In addition to the usual inpatient/outpatient coverage and emergency treatment, the plan includes:

    • Prescriptions (Rx Co-pay: $15 Generic/$40 Brand/$40 Non-Preferred)
    • Xray and lab costs (Covered per policy: 80% In-Network/ 60% Out of Network)
    • Preventive care/Wellness & Immunizations: (per policy- In-Network 100% of PA - Deductible Waived)
    • Mental illness and substance abuse (per policy) 
    • While traveling abroad, the plan also includes:

      • Emergency medical evacuation
      • Family emergency travel
      • Repatriation and return of mortal remains

    • Accidental death and dismemberment

Stetson Health Service requires that all students complete a Health History Form. For questions regarding the health history form or the immunization form, please contact Stetson Health Service at 386-822-8150.

Payment Options

Your enrollment form and premium payment must be received by midnight on September 12, 2015 in order to be covered by the Student Insurance 2015-2016 Annual of Fall-Only Plan.

Students will not be able to enroll for coverage after the enrollment grace period date above unless there is a qualifying event. Please contact CHP Insurance regarding definitions of a qualifying event.

  1. Pay via the online enrollment form with a credit card/debit card at the CHP Website: www.CHPStudent.com. Enrollment must be completed by midnight on September 12, 2015.
  2. Pay by check or money order by downloading a hard copy of the enrollment form at the CHP Website: www.CHPStudent.com. Mail in the enrollment form along with your payment to the CHP insurance agent (the mailing address is on the enrollment form). The agent must receive your enrollment form and payment by midnight on September 12, 2015.

Student Personal Property Insurance Coverage

  • Reasonable coverage just for college students.
  • Includes accidental damage, theft, hurricane, flood, fire, lightning surge, etc.
  • Student Plan is primary over parents' homeowners insurance, which often have high deductibles.
  • Protect your computer, cell phone and other electronics and valuables for as little as $100 for a year's coverage and deductibles as low as $25.
  • Coverage is available to students living on-campus and off-campus.
  • Coverage applies while gone on breaks and traveling abroad.
  • Coverage extends to bikes, jewelry and musical instruments up to $2,000.

Please note that as with any property insurance plan, make sure to document the property you are covering. Documentation can include purchase receipts, photos of the items and written records of the product name, model number, value/cost you paid and any applicable serial numbers. Keep this information in a file in case you need to make a claim.
