Free Wellness Assessments

The Holistic Lifestyle Questionnaire is offered through the National Wellness Institute. It's designed for college students and takes about 15 minutes to complete. It's an educational and awareness tool that covers the six areas of wellness: physical, social, emotional, occupational, spiritual and intellectual.
