Forms that must be completed and submitted to the Department:

• Request  for Letters of Recommendation (complete and submit one to each faculty member from whom you are requesting letters)
  

• Senior Project Title Form (complete at beginning of Senior Project semester and turn in to Department Office, 309 Flagler)
  
  -The Certification of Oral Communication Competency is available in the Department office only. Please ask your adviser to explain the certification process
  

For those using the Psychology 101 research pool:

• Research Information/Informed Consent (once a research project is approved, this form must be completed and posted with the researcher's sign-up sheet)
• Participant Sign-up Sheet (researchers use this form to post dates and times available and put in on the experiment bulletin board for participants to sign-up)
