Alumni ID Enrollment Form

Please fill out this form only if you are:

Renewing an expired card (for cards purchased before July 1, 2025).
Requesting a new ID card for yourself and/or your dependents for the first time.
Replacing a lost card.

Please note: The ID card fee will apply again for all replacements.

Reminder: Cards purchased or renewed on or after July 1, 2025 will be issued indefinitely and will not be printed with an expiration date. You will only need to complete this form again if you need a replacement.

Once the below form is submitted, please allow 1-2 business days for processing. You will receive an email letting you know that you can pick up your Alumni ID card in the Hatter1Card office located in University Hall, room 142.
