This form must be completed at least 10 days prior to the event date.
The sponsoring organization/individual must obtain approval and signature from their department’s Vice President or Dean on the event alcohol registration from prior to submitting it for final approval to the University Events office. The sponsoring organization/individual will comply with Florida State Laws, local ordinances, and Stetson University's policies and is accountable for the actions of the guests at this event, therefore assuming all responsibility for any damage to University property.

As a Vice President or Dean of the sponsoring organization, I agree that the above requirements have been met and accept responsibility on behalf of the organization. A Director in my organization or the designated representative named below will remain in attendance throughout the duration of the event and will adhere to the University Alcohol Policy and Guidelines.
