| Immunization Requirements |

Students must provide a completed Medical History Form that includes proof of immunity to Rubeola and Rubella (Measles) before registering for classes. The Hepatitis B Series and Meningitis Vaccines, although not required, are highly recommended. Residential students are required to have the Hepatitis B series and Meningitis vaccine or a signed waiver stating they understand about the diseases and the availability of the vaccines, but have opted not to be vaccinated. The waiver is an online form located on the Housing & Residential Life website under the housing forms section. You will be required to submit this waiver to Housing & Residential Life in order to be assigned a room on campus. |
