S.A.F.A.C. Forms
S.A.F.A.C. Application - The S.A.F.A.C. Application is an electronic formthat will include all information regarding events that your organization wishes to apply for. Applications are submitted electronically through OrgSync and will not be accepted past the deadline.
Funding Request Form - This form is used for all requests for puchases to be made off-campus. This includes requests for the Wal-Mart, Publix, and Office Depot credit cards and Department Purchase Orders (DPOs) for outside vendors. Requests on-campus can be made with the using an On-Campus Request Form located only in the S.A.F.A.C. Office. Request form must be filled out prior to all purchases.
Information Packet - The information packet is primarily made available at all information sessions. This includes important details about the S.A.F.A.C. process, how to fill out the S.A.F.A.C. application and details about the post allocation process. There is also a sample application example.
Price Reference Sheet - The price reference sheet is intended to accompany the information packet. This is general price reference sheet and should not be the sole source of research done for events presented on the application. All prices are expected to be accurate and well researched prior to turning the application.
Spending Procedures and Policies - The spending procedures and policies provides a detailed guideline for spending S.A.F.A.C. funds. It is highly recommended that this form is carefully reviewed before spending any approved funding.
Example of a Consent Form - The S.A.F.A.C. Consent Form is an agreement between your organization and the S.A.F.A.C. Office on ethical use of funding. The form requires one person from your organization to be the primary S.A.F.A.C. representative and contact. This form must be up-to-date and turned in before the hearing date. Forms are only available in the S.A.F.A.C. Office and will not be available the day of the hearing. The attached form is intended to be a reference and cannot be used to fill out.
