These forms are PDF forms which can be printed and either faxed (386-822-7146) to our office or mailed (421 N. Woodland Blvd., Unit 8298, DeLand, FL 32723).
Re-entry Form- If you are planning to re-enter Stetson. You can use this form to send to the Registrar's Office.
Transient Student Form - If you are planning on taking courses at another college or university during the summer, or any term. Fill out form and bring to Registrar's office if on campus, or fax to us (386-822-7146) if off campus.
TRANSCRIPTS: IMPORTANT: The University Registrar's Office will be closed from Friday, Dec. 23 to Jan. 3. No transcripts will be prepared during that time. For transcripts to be sent before the holiday, orders must be received in our office by 4:30 pm on Wednesday, Dec. 21. After that, transcripts will next be sent on Tuesday, Jan. 3.
Stetson University has authorized the National Student Clearinghouse to provide transcript ordering via the Web. If you are a current or previously enrolled student you can access this link through your my.stetson.edu account by choosing the Student Services menu.
Transcripts are ordered using any major credit card. Your card will only be charged after your order has been completed. This service will allow you to track your request and will let you know when it has been processed.
Request for Transcript - You may still use this form to request a transcript; however tracking services are not available.
GRADUATION:
You must complete an Application for Graduation at the beginning of the semester in which you plan to graduate. There is a graduation fee that must be paid at the time of application. Your options to apply are:
Print and complete the PDF version you can fax this form to us at 386-822-7146 if paying fee by credit card. Otherwise application must be mailed to 421 N Woodland Blvd., Unit 8298, DeLand, FL 32723.
Complete the online form if you wish to pay online with a credit card using Stetson University's secure credit portal.
