Student Health History Form

All students are required to submit the Student Health History form.

Immunization Form

Students should print a copy of the Immunization form for their doctor to complete and sign. Upon completion it should be mailed or faxed to the Stetson University Student Health Service office using the address or fax number listed on the form.

Release of Medical Information Form

Students who need a copy of their immunizations to be sent to another university should complete the Release of Medical Information form. Upon completion it should be mailed or faxed to the Stetson University Student Health Service office using the address or fax number listed on the form.

Immunization Exemption Form

Students requesting an exemption from fulfilling the required vaccination(s) for religious reasons must complete and sign the Immunization Exemption form. Upon completion it should be mailed or faxed to the Stetson University Student Health Service office using the address or fax number listed on the form.
