Immunization Requirements

University Immunization Policy

Students must provide a completed Immunization form that includes proof of immunity to Rubella and Rubeola (Measles), also known as MMR vaccine, Hepatitis B and Meningitis Vaccines are required before registering for classes.

Residential students are required to have the Hepatitis B series and Meningitis vaccine or a signed waiver stating they understand the diseases and the availability of the vaccines, but have opted not to be vaccinated.

All completed immunization forms should be submitted to Stetson University Student Health Service, 421 N. Woodland Blvd. Unit 8415, DeLand, FL 32723 or faxed to 386-822-8152.

University Immunization Non-compliance Policy

Students who do not provide the required Student Health Form and Immunization Form will not be able to register for classes.

University Immunization Exemption Form

Students who are are requesting an exemption from fulfilling the required vaccination(s) for religious reasons must read and sign the Immunization Exemption form. Completed exemption forms should be submitted to Stetson University Student Health Service, 421 N. Woodland Blvd. Unit 8415, DeLand, FL 32723 or faxed to 386-822-8152.
