Be Prepared

 

Before you arrive at Stetson:

-Review your insurance policy, understand your insurance plan and all of its coverages as best you can.

 

 

When you Arrive at Stetson:

You should have the following:

-Thermometer

-Pain reliever/fever reducer (Tylenol/ibuprofen)

-Cold/flu relief (Dayquil/Nyquil, cough medicine, etc)

-Triple antibiotic ointment

-Bandaids

-Benadryl

-TUMS

-Inhalers and/or nebulizers if asthmatic

-Insurance card (policyholder's name and date of birth)

 

 

When you Arrive at Health Service:

 You should have the following:

-Please have your insurance card. If you are not the primary policy holder, please have primary policy holders' full name, date of birth and billing address.

-Please have a form of Identification: Drivers License, Military ID, Passport, ect. 

-Please have your social security number.

-Please have a complete list of Currently perscribed Medications (including dosages).
