Student Health Service Forms
The Student Health History Form and Immunization Form are required for all students. Please complete the Health History Form and submit online.
Students who wish to print a copy of the Immunization Form that their doctor can fill out, can do so through the link provided below. Upon completion the Immunization Form must be submitted to Stetson University Student Health Service, 421 N. Woodland Blvd. Unit 8415 Deland, Fl 32723 or faxed to (386) 822-8152.
Students who require a copy of their immunizations to be sent to another university should complete the Release of Immunization form and mail or fax it to Student Health Service.
Students who are requesting an exemption from fulfilling the required vaccination(s) for religious reasons, must read and sign the Immunization Exemption form. The form should be mailed to Stetson University, Student Health Service, 421 N. Woodland Blvd. Unit 8415, DeLand, FL 32723, or faxed to (386) 822-8152.
